Dudes Baseball Player Code of Conduct Agreements (Please check each box to confirm)

I will attend every practice (15 mins early) and game (a minimum of 60 mins early) on time (unless notified by a coach) and will notify the coach with minimum of 3 days notice if I cannot.
I will give best effort at all times on the field by running hard on all balls in play, sliding on close plays, and sprinting between the foul lines on when on offesne or defense.
I understand that I am ultimately responsible for my own self-confidence and agree to seek assistance and accept help regarding self-confidence and mental health.
I agree to practice my baseball skills on my own time and agree to provide video proof of self-led practice sessions upon request.
I understand that coaches may assign homework, such as watching baseball related videos and providing a written summation of the content if requested.
I understand that making outs, making errors and bad calls by the umpires are part of baseball. Therefore, I will not, pout, stomp my feet, throw my equipment, display negative body language or become innattentive to the game, in response to adversity during the game.
I will do my very best to listen and learn from my coaches as well as practice honesty and accountability at all times.
I will treat all players, coaches, umpires, parents, and spectators with respect, as I would like to be treated, using appropriate language in appropriate tones when interacting with them.
I will play for the team, not myself, I understand that playing baseball is considered a privilege and not a right.
I deserve to play in an environment that is free of drugs, tabacco and alcohol at the field and expect adults to also refrain from their use at all Dudes Baseball events. Any use of drugs, tabacco or alcohol inferred or otherwise, is grounds for immediate release.
When I arrive to the field for games or practices I will place my glove, with a baseball inside, on the foul line behind the first or third base bag and then immediately seek to assist in bringing gear to the field and setting up equipment without being asked.
I will clean up trash around the bench and spectator areas after each game and ensure the completion of the cleaning process before leaving the field by checking with a coach if the cleaning process has been completed.
I agree to maintain an appropriate academic standing as determined by my parents to be conisdered eligible for any Dudes Baseball event.
Players or Family members are not allowed to discuss scoring decisions with the GameChanger volunteer at any time. Appeals may be made directly to Colin McBride (General Manager) any time after the conclusion of the game.
Players and families are required to immediately notify a coach about all incidents that may impact sports performance i.e., concussion, injury, alcohol, and drug abuse, regardless of when, where, and how they occur.
Players are responsible for wearing exclusively team issued gear and maintaining all team issued athletic equipment and apparrel (including all three footware types, metals, molded and flat bottom shoes) and may be required to return team issued gear upon completion of the season or upon request from the General Manager. Players who leave a program in poor standing (quitting, release, etc.) will be required to return all equipment and apparel issued by Dudes Baseball.
I agree when "in-the-hole" (three batters away from hitting) I am responsible for retrieving the bat of the current hitter after the ball has been put in play.
I understand that adaquate knowledge and demonstration of the signs and of when and how to use a "walking lead" during the game are prerequisites for participating on offense..
I agree to assist in picking up my teammates on defense and retrieving foul balls immedatiely when requested by a coach or teammate even if I am currently on the bench.
Baseball will be my primary sport and highest athletic priotity between the months of May through July
Players and all Family members are a representatives of the Dudes Baseball program at all times and are prohibited from making any sort of public displays of defamatory statements about players, other teams, Dudes Baseball, or it's staff members, as well as on any social media sites.
I UNDERSTAND THAT THIS FORM INDICIATES MY COMMITMENT TO THE ENTIRETY OF A BASEBALL SEASON. MY COMMITMENT IS NOT BASED ON A WEEKEND TO WEEKEND BASIS.
I UNDERSTAND I MUST SECURE A PLAYER RELEASE FORM OR OTHER FORM OF CONFIRMATION FROM COLIN MCBRIDE (GENERAL MANAGER) TO PLAY FOR ANY OTHER BASEBALL TEAM DURING THE PERIOD THAT THIS CONTRACT IS VALID FOR.
I understand the tuition fee DOES NOT guarantee any set amount of playing time or position. It is exclusively earned through attendance, effort and performance.
I understand Dudes Baseball does not offer refunds of the team fee for ANY REASON WHATSOEVER

Violations of the Player Code of Conduct:
(Applied at the discretion of the coaching staff, our goal is to NOT have to resort to the steps below)

First Offense – Verbal warning and running.

Second Offense – Player will sit out of practice or game.

Third Offense – Meeting with coach, player, parent(s) and General Manager (Colin McBride).

Fourth Offense – Release of player without refund

Dudes Baseball Medical Release, Release of Liability, Photographhy and Media Release, Communication Consent, and acknowledgment of rules.

In consideration of being allowed to participate in any way in Dudes Baseball, its related events, and activities, I (the undersigned) acknowledge, appreciate, and agree that:

The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular skills, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and,
I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others, and assume full responsibility for my participation; and,

I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the General Manager (Colin McBride) immediately; and,

I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS DUDES BASEBALL, its officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used for the activity ("Releases"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to person or property associated with my presence or participation, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

Photography and Media Release:
I grant permission for photographs, videos, and/or player information collected by Dudes Baseball to be used for promotional purposes, including social media and other media outlets, without compensation.

Communication Consent
I authorize Dudes Baseball to contact me and my family with tournament information, marketing materials, program updates, special offers, and promotions via email, phone, text, and mail. I understand I can opt-out at any time by contacting Dudes Baseball.

Acknowledgment of Rules:
I acknowledge that I have read, understand, and agree to abide by the rules and regulations of Dudes Baseball. I certify that the information provided is accurate.

Do hereby consent to any medical care and the administration of anesthesia determined by a physician to be necessary for the welfare of my child while said child is under the care of Dudes Baseball, and I am not reasonably available by telephone to give consent.
